Driver Services Facility Checklist



What do you need to take with you to obtain a Driver's License?

Drivers ages 16-17:

Drivers ages 18 and older:

  • Meet all eligibility requirements, which are available in the Rules of the Road
  • Proper identification documents
  • Liability Insurance Card (if taking drive exam)
  • Knowledgeable about Rules of the Road
  • Payment of appropriate fee via cash, check or Discover credit card

What do you need to take with you to obtain an Illinois license plate?

All drivers:

  • Review what plates are available in our License Plate section.
  • You will have to fill out the Application for Vehicle Transaction(s) (form VSD 190). If applicable, the certificate of title issued by another state must be provided. Other documents must be provided to confirm information including name and address, odometer reading and vehicle description.
  • If there is an outstanding loan and the title is being held by an out-of-state lienholder, a copy of either the title, Certificate of Origin, out-of-state registration, or Bill of Sale and a copy of the loan agreement must be submitted in lieu of the title.
  • Some license plate/vehicle registration categories require proof of financial responsibility, proof of vehicle inspection certification, proof of disability, proof of disabled veteran status, proof of EX-POW status, proof of Congressional Medal of Honor recipient status, proof of National Guard status, or legible photocopy of FCC License.
  • Vehicles titled and registered in another jurisdiction for three months prior to moving to Illinois are exempt from Illinois tax, but Illinois Department of Revenue tax forms RUT-50 (if purchased from an individual) or RUT-25 (if purchased from an out-of-state dealer) must be completed showing exempt status. If you leased or rented a vehicle for three months or more and lived out of state, we will need a bill of sale. (All Illinois taxes must be paid when the vehicle is leased.)
  • Vehicles titled and registered in other jurisdictions for three months or more prior to relocating to Illinois are exempt from Illinois Revenue Used tax. The Illinois Department of Revenue requires that a RUT-50 (if purchased from an individual) or RUT-25 (if purchased from an out-of-state dealer) be completed showing exempt status. If you have a leased or rented vehicle, you will need the bill of sale and a copy of the lease agreement. In addition, for rented vehicles, a copy of the certificates of insurance is required.
  • Under Illinois law, your vehicle must be covered by liability insurance that covers the property damage and/or injuries you may cause others in an accident.
