Lost or Stolen Drivers License



Contact your local police department as soon as possible to report your lost or stolen driver’s license. 

If you are in Illinois, a duplicate driver's license can be issued at any Secretary of State facility in the state. You will need to present acceptable identification and pay a fee.

If you are out-of-state, contact the License & Medical Review Section at (217) 782-2720 for information on receiving a temporary driver’s license and/or an application for a duplicate driver’s license to be mailed to you at your out-of-state address.

If you are out-of-state, you may also request a temporary driver’s license and/or an application for a duplicate driver’s license by e-mailing our office with your full name, date of birth, Illinois driver’s license number and out-of-state address.

If you fear you may be a victim of identity theft, the Federal Trade Commission (FTC) has developed an Identity Theft brochure that permits identity theft victims to report information to many companies using just one standardized form.
