Persons with Disabilities License Plates are available to persons who have qualifying disabilities and have submitted a physician's certification form.

  • Current plates expire within 90 days - $107

To Apply

  • one must have a Persons with Disabilities Certification, signed by a licensed doctor.
  • The request can only be processed in Springfield. Apply in person or mail the proper documentation to:

Availability

  • Persons with Disabilities license plates are multi-year and may be displayed on passenger vehicles, trucks and vans weighing 8,000 pounds or less and Sport Utility Vehicles.
