Important Facts
- No refund or adjustment will be made for a change of choice or spacing after a plate has been ordered, or if the request
form is incorrectly completed.
- The Secretary of State’s office is authorized to recall a plate that is determined to be obscene or offensive.
- All request forms should indicate whether or not you want the Secretary of State to assign one of your choices with the next available number if your exact request is unavailable.
- If you wish to submit more than three requests, attach additional selections to the request form.
- Numbers are required to precede letters on personalized B-Truck, RV, RT, Antique Vehicle and Antique Motorcycle plates.
- If you want a space between letters, please indicate with a slash mark. An automatic space isrequired between letters and numbers. Spacing varies with the type of plates requested. Propercompletion of the form is the customer's responsibility.

- Use characters, symbols, foreign words or symbols of punctuation, such as dashes or periods

Agriculture in the Classroom, America Remembers, Antique Vehicle, B-truck, Collegiate*, Eagle Scout, Education, Environmental, Firefighters Memorial, Illinois Route 66, Illinois Pan Hellenic, Law Enforcement Torch Run for Special Olympics, Mammogram, Master Mason, Organ Donor, Park District Youth Program, Passenger, Persons with Disabilities, Pet Friendly, Police Memorial, Prevent Violence, Recreational Vehicle (up to 8,000 lbs.), Sheet Metal Workers International Association, Sporting Series and Support Youth Golf.
- A $29 replacement fee is assessed to all plate switches, with the exception of Motorcycle and Recreational Trailer (RT) plates, which have a $26 replacement fee.
- A $13 surcharge is assessed to the renewal fee.

Military License Plates—Afghanistan Campaign Medal, Armed Forces Reserves, Armed Forces Retired, Bronze Star, Disabled Veteran, Ex-Prisoner, Gold Star, Illinois National Guard, Iraq Campaign Medal, Korean War Veteran, Marine Corps, Pearl Harbor Survivor, Purple Heart, Silver Star, Universal Veteran, Universal Veteran Motorcycle, Vietnam Veteran, and World War II Veteran do not require the extra-personalized license plates fee.
