How do I obtain a Certificate of Good Standing?
You may order a Certificate of Good Standing from the file detail report of a corporation or limited liability company--providing, of course, that the entity has satisfied all requirements of the relevant Illinois statutes. You may then immediately print out the certificate once the purchase has been completed. For both a business corporation and a limited liability company the fee for the certificate is $27.00. For a not-for-profit corporation, the fee is $6.00. Payment of the required fee may be made by the following credit card types: Visa, MasterCard, Discover, or American Express.
You may also purchase a Certificate of Good Standing by calling (217) 782-6875. The fee is $25.50 for credit card transactions (Visa, Discover or MasterCard). You may also submit your request in writing along with a check for $25.00 to Illinois Secretary of State, Corporations Division, Third Floor Howlett Building, Springfield, IL 62756.
How do I obtain a copy of a corporation's annual report?
Submit your request for an annual report in writing along with a check for $25.00 to Secretary of State, Corporations Division, Third Floor Howlett Building, Springfield, IL 62756. To order by phone, please call (217) 782-6875. The fee is $25.50 for credit card transactions (Visa, Discover or MasterCard). The annual report contains the names of the officers and directors of the corporation or company, but ownership is not on record with this office.
How can I check the status of a corporation or get incorporation forms online?

How is a limited liability company organized in Illinois and how do I obtain information on them?
To organize a limited liability company in Illinois, you must complete and submit to this office Form LLC-5.5, Articles of Organization, which can be downloaded from our Web site. Sole proprietorships are filed only at the county level. Check with your county recorder's office to determine their filing requirements.
Information on limited liability companies (LLCs) is available to the public from the Department of Business Services upon written request, by telephone or in person. I represent a company doing business in another state. Do I need to complete any forms to transact business in Illinois? Yes, the appropriate form must be submitted to the Secretary of State. A corporation must complete an Application for Authority to Transact Business in Illinois (form BCA-13.15) while a limited liability company must complete an Application for Admission to Transact Business (form LLC-45.5). They should be submitted in duplicate and accompanied by the appropriate certified copy of the Articles of Incorporation and any amendments thereto or Articles of Organization, also as amended, from the state where you are presently organized.
How can I reinstate my corporation?
You must file an Application for Reinstatement (BCA 12.45) in duplicate and submit it to the Secretary of State. You also must file annual reports for the years that were not filed. If you have questions regarding the cost of reinstatement, please call (217) 785-5782.

Driver's License
I have lost my driver's license. How can I get a replacement?
First, please contact the local police department to report your lost or stolen driver's license.
If you are in Illinois, a duplicate driver's license can be issued at any Secretary of State facility in the state. You will need to present acceptable identification and pay a $5 fee. For examples, please refer to the complete list of acceptable documentation.
If you are out of state, please call the Special License and Re-Examination Unit at (217) 782-2720. If you plan to return to Illinois within 90 days, you may request that a 90-day temporary driver's license be mailed to you at your out-of-state address. If you will be out of state for more than 90 days, the Secretary of State's office will mail you the forms to apply for a non-photo duplicate driver's license.
You also may request a duplicate driver's license packet by e-mailing this office with your full name, Illinois driver's license number, date of birth, out-of-state address and your date of return to Illinois.
I am out of state and my driver's license is coming up for renewal. What should I do?
If you are in the military and stationed outside the state of Illinois, you, your spouse and children living with you may have the expiration of your driver's licenses deferred for up to 90 days after your discharge or reassignment to a military base in Illinois. A Military Deferral Certificate, designed to be carried with the expired license, can be issued at no charge and mailed to you at your out of state address.
In order to obtain a military deferral, mail in a photocopy of your current Illinois license, military ID card, and proof of residence to the Special License Unit at the Secretary of State, 2701 S. Dirksen Parkway, Springfield, Illinois 62723. Please include your out of state mailing address. Acceptable forms of proof of residency are as follows:
- Illinois Vehicle Title
- Illinois Vehicle registration card
- Illinois Voter registration card
- Lease agreement (within 90 days and showing full Illinois address)
- Rent receipt (within 90 days and showing full Illinois address)
- Utility bill (within 90 days and showing full Illinois address)
- Bank statement (within 90 days and showing full Illinois address)
- Official postmarked mail (within 90 days and having legible postmark to Illinois address from a business) Note: Handwritten mail not acceptable

You also may be eligible to renew your driver's license through the Safe Driver Renewal Program.
It may be helpful to know that Illinois drivers can renew their licenses up to one year before a four-year or two-year license expires, or six months before a one-year license expires.
Is it true that the minimum driving age is being raised to 18? How can I find information on teen driver's licensing requirements?
The widespread rumor about a change in the minimum driving age from 16 to 18 is false. For complete information on licensing requirements and sanctions for teen drivers, please review the Illinois Graduated Licensing System on our Web site.
The Illinois State Board of Education administers high school driver education programs and issues blue slips. For information on these, please call the Board at (217) 782-5256.
How do I obtain a copy of my driving record?
The quickest way to obtain an abstract of your driving record is to go to your nearest Secretary of State facility, complete a request form, pay the $12 fee (no credit or debit cards accepted) and receive the record on the spot. You may also mail your request along with the $12 fee to the Secretary of State, Driver Analysis Section, 2701 S. Dirksen Parkway, Springfield, IL 62723. Include the full name, date of birth and Illinois driver's license number if possible. Please allow 10 working days for a response.
If you are the parent or legal guardian of a minor person (under age 18) and are requesting the driving record of that minor person, the requestor must submit a notarized written permission of the person listed on the request form to obtain the driving record and pay the $12 fee.
If you are an immediate family member (parent/legal guardian, brother, sister, spouse, grandparent, child or grandchild) of the adult person (age 18 or older) and are requesting the driving record of that adult person, the requestor must submit a notarized written permission of the person listed on the request form to obtain the driving record and pay the $12 fee.
If you are requesting a driving record on another person, please allow an additional 10 days for processing. Before any information is released on another individual, notification will be sent to that person, giving the date and name of the person making the request.
If you are out of state, a family member (parent/ legal guardian, brother, sister, spouse, grandparent, child or grandchild) in Illinois can obtain a copy of your driving record at a Driver Services facility. The requestor must submit a notarized written permission of the person listed on the request form to obtain the driving record and pay the $12 fee.
The Illinois Secretary of State no longer issues clearance letters. If another state insists on written proof of your driving status in Illinois, you may request a certified copy of your driving record for the licensing authorities in that state. My driver's license is suspended. Can I apply for a permit to drive to work or school?
To apply for restricted driving privileges (RDP) during the period of suspension, you should contact an informal hearing officer at a Secretary of State facility. The hearing officer will review your driving record and explain the requirements and procedures to apply for driving relief. Please call the facility to determine the availability of the hearing officer. All informal hearings are held on a walk-in basis.
My child has an Illinois instruction permit. Can he/she drive in other states while we are on vacation?
Illinois law allows a person with a valid instruction permit issued in another state and of the proper class to drive a vehicle when traveling through our state.
Also, most states reciprocally recognize the Illinois instruction permit. You may, however, wish to contact the local law enforcement agencies of the state in which you will be traveling.
I will be working in/moving to Illinois. How do I obtain a driver's license in your state?
Drivers moving to Illinois may use their valid driver's license from their home state or country for 90 days. You may obtain an Illinois driver's license or identification card only if you are becoming a legal resident of Illinois. If you have a valid driver's license from another state or country, you may use it to drive in Illinois throughout your stay, if you do not plan to become a permanent resident of this state. Illinois does not recognize the international driver's license.
To receive an Illinois driver's license or ID card, you must provide acceptable identification to prove name, date of birth, Illinois residency, Social Security Number and your signature for comparison. For examples, please refer to the complete list of acceptable documentation.
To obtain an Illinois driver's license, you must successfully complete a vision screening, written examination and possibly a driving examination. The fee for an Illinois driver's license is $10 and is valid for four years. The fee for an Illinois ID card is $20 and is valid for five years.
How can I obtain a motorcycle driver's license?
If you wish to obtain a motorcycle classification on your driver's license, you must pass a separate motorcycle written and road examination. The two classifications are Class L for motor-driven cycles with less than 150 cc and Class M for motorcycles.
If you are age 18 or over, you may apply for a 12-month instruction permit to operate a Class L or Class M motorcycle. It allows you to drive only during daylight hours, under the direct supervision of a licensed motorcycle operator.
If you are under age 18 and have satisfactorily completed driver education, you may apply for a 12-month instruction permit to operate a motor-driven cycle Class L (under 150 cc). It allows you to drive only during daylight hours, under the direct supervision of a licensed motor-driven or motorcycle operator.
An instruction permit for a Class M license may be issued to an applicant under age 18 only after successful completion of an approved driver education course and after enrollment in an Illinois Department of Transportation (IDOT) Motorcycle Rider Education course. To initially obtain a Class L or Class M license, a person age 18 or older who successfully completes an IDOT motorcycle training course will not be required to pass a driving or written examination at a Secretary of State facility.

Payment Processor Fee
Why do we have to pay different payment processor fees depending on how we choose to renew?
The Secretary of State's office must pay a small fee per transaction to the credit card providers; the fee for each service was set by our financial institution. License plate renewal fees are set by state statute and we are unable, by law, to absorb the cost of the additional provider fee at this time.

I renewed online but now need to cancel that order (I sold the car, My husband bought a sticker through another renewal method, I'm moving out of state).

I accidentally hit the Submit button twice on the payment page. Can you credit my account or let me know that I was only charged once for my online renewal?
Please check with your credit card company to determine whether you were charged more than once for your vehicle registration renewal.
I do not have the renewal form and need a PIN and Renewal Code.

I ordered online and now I've moved/I'm out of state temporarily/I need my plates sent to a different address. What do I do?
To protect citizens against fraud and abuse, our policy is to ship license plates and stickers to the permanent Illinois address in our registration files at the time of order only. (The only exceptions are military and college students.)
To cover all of the bases to ensure that you will receive your plates soon, please do the following things:
- Contact your local post office and tell them that you have been expecting a plate shipment. You may want to contact the post office from your previous address as well. Also, fill out the appropriate Forwarding form with the Post Office. This form should ensure that your new plates arrive at your new address.
- Submit your new address to the Secretary of State's office Online Address Change. This will update both our vehicle registration files and driver's license files.
- If your plate order does arrive back in our office for a better address, we will immediately send it out to the new address you provided.
I just renewed online. After I completed my registration, I realized that I could have elected to purchase an organ donor plate. Although I already completed my online renewal, can I make a change & order the Donor Plate?
Unfortunately, we cannot change your renewal order that is already being processed by our system. You should affix the sticker/plate that is sent to you as proof of valid registration to your vehicle while you are seeking to obtain an organ donor plate.
To receive an organ donor plate, you must fill out a Request Form, which is available online. The Vehicle Services Department will notify you by mail regarding your request. You will have to pay the additional organ donor fees, plus a $29 charge for the manufacture of the new plate and sticker.
When you receive your organ donor plates, we encourage you to recycle or destroy and dispose of your other plates.
I was trying to renew online but my credit card wouldn't go through.
Please be sure that you are using a valid Visa, Mastercard, Discover or American Express card that has not expired and that you have correctly typed in your numbers. You can not use a debit card for this transaction. If everything is correct, it is possible that the verification system is currently unavailable and we suggest that you wait a few hours and try to access the system again.

One of my license plates is missing. How can I replace it?

How can I get the Walter Payton Organ Donor License Plate?
The Organ Donor License Plate is in honor of the late great Chicago Bear Walter Payton. Following are options for purchasing a set of the plates:
- You may apply for the plates at the time your current plates expire by submitting a
- request for Organ Donor Plates,
- registration renewal application and
- $103 fee payable to the Secretary of State ($78 registration fee and $25 original issuance fee).
- You may apply for the plates up to 90 days before your current plates expire by submitting
- request for Organ Donor Plates,
- completed registration application,
- current registration ID card
- $54 fee payable to the Secretary of State ($25 original issuance fee and $29 replacement plate fee).
- For a newly acquired vehicle, the $25 original issuance fee and $78 registration fee are required, along with a $65 title fee.

Mistakes on Registration Card
I noticed that the spelling of my first name/Drivers License number/other information is incorrect.
If there is a slight error in the spelling of your name on your registration card, such as "John Dqe" instead of "John Doe," please email us using the License Plate and Vehicle Registration contact form and we will correct your registration. If the spelling is more severely wrong, or you have changed your name due to marriage, etc., you must visit a Secretary of State Facility to prove your identity and update your information. There may be fees involved in updating your records. See Vehicle Registration/Renewal Stickers in this section for more information.
If your Driver's License number is wrong or not listed on the Registration card, you can email the correction to License Plate and Vehicle Registration contact form.

I just renewed online, and I just realized that on the registration it says it needs my driver's license number. What do I do?
Please email us using the License Plate and Vehicle Registration contact form and include your Drivers License number so we can update your registration file.
Vanity or Personalized Plates
I have sent in my request form for a vanity or personalized plate, but have not heard back from your office. What should I do?
You should receive a letter in the mail letting you know whether you have been awarded the plate or denied the request based on availability. If it has been longer than six weeks since you mailed in your request, please contact us.
I received the award letter for a vanity or personalized plate and returned it to your office with the appropriate fees. Where are my plates?
Once our office receives your award application and appropriate fees, it takes four to six weeks to process and order your plates from the manufacturer. At that time, we will send you a temporary sticker to affix to your current plate until your new vanity or personalized plates arrive. (It takes five to ten business days for the sticker to travel by mail.)
If your current plates will expire in the next two weeks and you have submitted your renewal for your current plates with your application and you have NOT received a temporary registration sticker, please contact us.
We suggest you fill out the online request form today. It will take four to six weeks for Vehicles to check whether your selections are available and send you a letter regarding the outcome. You have 45 days within which you can return the letter, the appropriate fees, and claim your plate.
Renewal notices are sent out 60-90 days before your plates are due to expire.
Vehicle Registration/Renewal Stickers
I have not received my license plate renewal sticker and I mailed in the fee more than 30 days ago. What should I do if it doesn't arrive in time?
All renewal applications received by this office are prioritized and processed in expiration date order. For example, if your registration sticker expires the last day of April and you mailed your renewal form and fee in February, priority will still be given to those renewals that expire in March.
It should take five to ten business days for renewal stickers to arrive from the time your mail-in renewal hits our office.
To inquire about the status of your renewal application, please email us using the License Plate and Vehicle Registration contact form or call (217) 782-6387. You may first wish to contact your banking facility to determine whether the check used for payment has been cashed.
I did not receive my license plate sticker renewal notice. How do I renew?
The license plate registration renewal notice is sent out as a courtesy reminder, and it is the driver's responsibility to ensure that their annual license plate sticker is current. You may visit your local Secretary of State Facility to renew your license plates, or submit a letter indicating your name, address, license plate number and a daytime phone number with a check for the appropriate renewal amount to:

Standard processing time is 30 days. Annual renewal fees for standard license plates are $78. The renewal fee for personalized plates (combination of letters and numbers) is $85.The renewal fee for vanity plates (all letters or all numbers) is $91. For the fastest service, please visit your nearest Secretary of State facility to renew your registration and receive a sticker on the spot. Currency exchanges and participating banks will also process registration renewals (minimum $3 service fee charge). Some of these entities will accept last year's registration card in lieu of the renewal form. Please call ahead to verify the service charge and make sure the institution/exchange will handle the sticker transaction without the renewal notice.
What is the Circuit Breaker license plate registration fee discount, and how can I determine if I qualify?
The Circuit Breaker Program is administered by the Illinois Department of Aging to assist senior citizens and persons with disabilities. Under the program, there are three main benefits, one of which is a license plate renewal discount through the Secretary of State. The discount fee for your license plate is as follows:
- Passenger, Persons with Disabilities, Hearing Impaired, B-Truck or Recreational Vehicle (8,000 pounds or less), Amateur Radio, Armed Forces Reserves, Bronze Star, Gold Star, Illinois National Guard, Korean War Veteran, Pearl Harbor Survivor, Purple Heart, Retired Armed Forces, Silver Star, Universal Veteran, Vietnam and World War II Veteran. Annual Renewal $78 ($24 your fee)
- Personalized Passenger, Persons with Disabilities, B-Truck and Recreational Vehicle (8,000 pounds or less). Annual Renewal $85 ($31 your fee)
- Vanity Passenger, Persons with Disabilities, B-Truck and Recreational Vehicle (8,000 pounds or less). Annual Renewal $91 ($37 your fee)
- Marine Corps. Annual Renewal $98 ($44 you fee).
To qualify for the Circuit Breaker Program, applicants must be at least age 65, or at least age 16 and totally disabled. A person needs to be an Illinois resident and have an annual income of less than $21,218; or a combined annual income of $28,480 for a two-person household; or a combined annual income of $35,740 for a three-person household.

My registration renewal notice has incorrect information on it. Should I correct it on the form?
For address changes, mark the box on the front of the form denoting address change information. Then fill out the Address Change boxes on the back of the form.
You may renew using CyberService Express to renew your sticker and update your address, or visit a nearby Secretary of State facility.
If the vehicle information is incorrect because you recently bought a new vehicle, please make the changes and attach a copy of the transfer ID,reflecting the transfer of the license plates to the new vehicle. If you have not received the transfer ID card, you may go ahead and mail in the registration form as is with your fee. When the transfer ID card arrives, attach it to the new registration card you receive with your renewal sticker.
If the driver's license/FEIN number is incorrect, line out the number and write in the correction. If the driver's license/FEIN number has been omitted, you should write in the driver's license number. Do not write in out-of-state driver's license number or a driver's license number for a company-owned vehicle on the registration form.
To change a name on a registration, you must visit a Secretary of State facility to correct your title before renewing your registration. Application for Vehicle Transaction(s) (VSD-190) must be completed and submitted with the $65 title fee, the $3 corrected registration ID card fee, and the documentation. Proper documentation includes a copy of a marriage certificate, a driver's license (with corrected name) or other court document.
I lost my registration sticker/card. How can I obtain a new one?
You may purchase a replacement sticker and/or duplicate registration card at a nearby Secretary of State Facility. If your registration has already been renewed, a replacement sticker can be purchased for $20. The fee for a duplicate registration card only is $3. To apply by mail, submit your request with your name, address, plate number and appropriate fee to:

If I've sold my vehicle, what should I do with the license plates?
If the vehicle will not be replaced, the license plates should be removed and destroyed. If the vehicle will be replaced, you may transfer the license plates to the newly purchased vehicle, as long as both vehicles fall under the same plate category.
I've received a renewal notice for a vehicle that I've sold, what should I do?
If you no longer own the vehicle and you are not planning on replacing the vehicle and transferring the license plates, you may simply destroy the renewal notice.

Why did I receive a letter from the Secretary of State requesting proof that my vehicle is insured?
Illinois' insurance law is considered to be one of the most successful laws of its kind in this country. This success is directly related to our unique enforcement approach that includes a questionnaire sampling process and the issuance of traffic tickets.
The questionnaire sampling process involves sending insurance questionnaires to vehicle owners chosen randomly by computer from the registration files. The vehicle owner is required to respond with the name of their insurance company and the policy number. If this information is provided by the vehicle owner, the named insurance company is asked to verify coverage.
If a vehicle owner is not insured on the verification date stated on the questionnaire, or if the vehicle owner fails to respond to the questionnaire, he/she is considered to be in violation of the mandatory insurance law. As a result, a notice of suspension is mailed to the vehicle owner. Read more about the Mandatory Insurance Law and penalties.

What is the I-PASS and how do I obtain one?
The Illinois Secretary of State does not issue the I-PASS.
I-PASS is the Illinois State Toll Highway Authority's electronic toll collection system. This system allows you to pay tolls electronically, therefore eliminating the need to carry change, roll down your window, wait for lights to turn green, and in some instances even eliminates the need to stop your vehicle.
