Illinois Procurement Ethics Law (PA 95-971), Vendor Certification Requirements

In order to receive a payment from the Secretary of State, Individual/Vendors must complete a Vendor Certification Form. Please complete the form and if required, include a copy of the certificate issued by the State Board of Elections and return to Budget & Fiscal Management, Room 124 Howlett Building, Springfield, IL 62756.
